Liability Certificate of Insurance
The Certificate of Insurance request form is used to formally request a Liability Certificate of Insurance (COI) from Brown University departments. Please complete the form and keep a copy for your records. If this request is related to the use of facilities outside of Brown, you must ensure that the contract or rental use agreement has been appropriately reviewed.
If a party is asking to be named as additional insured, please indicate that on the request, and email a copy of the signed contract or provide the BCM contract number so that we are able to confirm that the agreement was reviewed and approved.
Request for Liability Certificate of Insurance
Contracts need to be reviewed and can only be signed by those who are authorized to do so. Insurance language in contracts needs to be reviewed and approved by the Insurance Office to ensure that the University can agree to the requirements included in the agreement. Fine Arts/Property Insurance Certificate of Insurance
The purpose of Fine Arts/Property Insurance Coverage is to cover University owned fine arts/property, or items that Brown is contractually obligated to provide insurance coverage for. There is a departmental deductible of $5,000 if items are damaged.
